I am about 2k short of hitting 200K.
I intend to undertake a major service to hopefully increase the car's longevity.

I am going to divide the jobs into 3 categories.
    1) Things I must replace even though they are working fine right now but do have age on them.
    2) Things that I could replace but if they are working fine keep an eye on them for a possible future replacement.
    3) Things that can be ignored.

I am not worried about regular items like oil/filters/grease/ and other consumables.
I am specifically looking at those parts that need to be replaced at this mileage.
    Current Mileage 198k
    For context:
I have brand new OEM suspension that I added in 2023 at about 179K.
    Serpentine belt changed at 162k
Original tentioner, pulleys, water pump, thermostat, alternator.
    Spark plugs at 162k
PCV, power steering fluid, brake fluid at 184k.
    ATF fluid at 162k
    Coolant at 162k.
    Fuel pump 154k
    Front wheel bearings 135k

What are the things that folks who have reached this mileage suggest I must change, nice to change and can ignore.
We can make this thread as a ready reckoner for everyone once people get up there in terms of mileage on their 5th Gen

    I did!
But that list mostly has consumable items .

I was hoping people who have reached that mileage could pitch in with parts that they had to replace out of the ordinary. For eg, the radiator cap. The cap does wear out over time and should be changed.

I'm approaching the same mileage. There's just not a lot that needs to be replaced. I'm going to do LCAs because the bushings are less than ideal, going with OEM. I've also got a re-manned steering rack sitting on my garage floor because I have a slow leak in mine. It's just not bad enough yet to justify replacement. I've done the same as you and replaced things as needed over the course of the life so far, and they are mostly consumables. These things are well built and don't need a whole lot. At 200K I'm pretty much going to follow the recommendations on the maintenance map and combine it with the owner's manual.

Not with 4Runner, but took other cars to high mileage. Some things not on your list to consider:

    1. Boots around CV get torn, dirt gets in and this kills your axles unless caught early or boots replaced preemptively. These tend to be expensive parts if you have to replace.
    2. Upper radiator hoses get brittle around ends, increasing chance of blow-out.
    3. Seals around brake caliper pistons get worn, usually there are kits for rebuilding.
    4. Sun roof drains get clogged, water gets all over interior. Big trouble.
    5. Mechanical ignition locks require lubrication or they start wearing down the key and can get stuck with age. You don't want to be drilling one out. Also take a hard look at the key, if it has ridges rounding off better order a new one.
    6. Spare tire, if never used, rots out. Don't assume it would be there if you need it unless you check on it periodically.
    7. Seat cushion foam can usually be purchased as a separate part and replaced with moderate difficultly. Doing so will make your seat more comfortable, as high-miles = high seat time.
    8. You can get rubber conditioner (I use Wurth) spray that will make your door seals soft again, this will reduce wind noise on the highway.

Nothing. You've already done more than I have ever had too. I did have to rebuild front axles which is uncommon before 300k in my experience, but one was operator error and the other one started leaking because I worked on a Sunday. Toyota #04427-60133 is front axle boot kit for $46/ ToyotaPartsDeal.com. The axle seldom goes bad, just the boot. You can drive quite a while on a busted boot. I don't want to stick a $32 Chinese axle in mine. It's $400 from Toyota and they are designed to last the life of the vehicle. I change everything every 100k and use synthetic everything, transmission and steering cooler, Mobil 1 75w90 synthetic gear oil works in transfer case and diffs. It's supposed to be good for 500k if you forget. I use distilled water with Toyota red and Toyota or Aisin WS trans fluid and change it every 60-75k. Letting coolant go to long and become acidic is damaging some higher mileage Toyotas they are finding. Eats head gasket between the cylinders and can't be rebuilt because of aluminum block with steel sleeves cast into it. Mostly 5.7 and 3.5s. The Car Care Nut on YouTube is an excellent Toyota master tech educator.
