None. Bone Stock. EXCEPT: Brushguard, tow hitch, both welded to the frame. It's good to have friends and a fully equipped garage!
Sounds like a head gasket to me.

I have feeling that you need to pull the head off. Either the head isn't torqued on sufficiently, it was torqued out of order, the head or the block had a piece of something still on when the gasket was placed, the head or block are warped, or the gasket was damaged while installing it. Or the head is cracked. I got a new head for my 87 4Runner, when a friend of mine blew the factory head, that was cracked to start out. Brand new, fresh for the manufacturer, it had a visible crack halfway across it.

Any of those would do it. I am going to presume you used a new gasket, so that isn't a consideration. Did you use new head bolts? Not necessarily essential, but I've read it's a good idea on GP. Are you certain the gasket is on right side up? Did you check the head and block, both, with a known-good straight edge to ensure they are within spec as far as flatness goes? "New" is no promise of "Good", as I showed above.

To my knowledge, there is no hose you can put into the wrong place to do what you're seeing. The cooling system is pretty isolated from the cylinders, oil system, and exhaust, other than at the head/block interface. I would start looking there. Or, the oil/coolant can mix if there's an oil cooler feed into the radiator that has broken inside the radiator. I don't believe it would come out the exhaust, though. Usually, it's the automatic transmission that has a feed through the radiator, though. I just saying that it might have an odd mod on it that runs oil through the radiator. Unlikely, but possible.

Anywho, let us know what you find!
